This is a sign up for a single 90 minute open pool session at the Pallottine Center pool in Florissant, MO.  This form must be completed for each attendee in your party (i.e. fill out a separate form for a child, spouse, friend, etc.). All minors under the age of 18 must have a guardian present during the pool session.
