If you are applying for MWA membership:
- Download the Membership Application link at the bottom of the page,
- Print and Complete the application with the necessary information,
- Mail application and required fees to the address provided in the Membership Application.
If you are applying for MWA and ACA membership:
- Download the Membership Application link at the bottom of the page,
- Print and Complete the application with the necessary information,
- Download the appropriate ACA Waiver at the bottom of the page,
- Complete the ACA Waiver information (for each person if this is a family membership),
- Mail ACA waiver(s) with MWA application and required fees to the address provided in the Membership Application,
- If you are an ACA Instructor please remember to add the additional SEIC fees,
- If you are joining the ACA the waiver(s) MUST be sent along with the MWA application.
